The
Good
Government
Act, 2009
requires
that voters
now provide
identification
prior to
obtaining a
ballot. The
following is
a list of
documents
that show
name,
qualifying
address, and
signature
(please note
that you
must provide
the original
copy of the
document):

an
Ontario
driver's
licence;
-
an
Ontario
Health
Card
(photo
card);
-
an
Ontario
motor
vehicle
permit
(plate
portion);
-
a
cancelled
personalized
cheque;
-
a
mortgage,
lease
or
rental
agreement;
-
-
a
loan
or
financial
agreement
with
a
financial
institution;
-
a
document
issued
or
certified
by a
court
in
Ontario;
-
any
other
document
from
the
government
of
Canada,
Ontario
or a
municipality
in
Ontario
or
from
an
agency
of
such
a
government;
or
-
any
document
from
a
Band
Council
in
Ontario
established
under
the
Indian
Act
(Canada).
